Registration Forms

All students are required by State Law to obtain a physical examination from a licensed physician prior to entrance into Early Childhood, 4 Year Old Preschool, Kindergarten, and 6th Grade as well as participation in middle school athletics. Transfer students are allowed thirty (30) days to comply with this requirements. A certified birth certificate is required upon initial entrance into District 10 Schools.
